The following documentation will be REQUIRED at registration for your child to be able to play ball.
Three forms of identification are required - see some options listed below.

1. One utility bill for current month
2. Current car registration
3. State Issued I.D.
4. Employment Records
5. Insurance Documents
6. Voter Registration Card
ALL THREE FORMS OF ID WILL NEED THE CURRENT ADDRESS AT WHICH THE PLAYER'S PARENT(S) (OR HIS/HER COURT APPOINTED LEGAL GUARDIAN) RESIDES.
A Birth Certificate is REQUIRED for each child being registered. AN ORIGINAL DOCUMENT ISSUED BY FEDERAL, STATE OR PROVINCIAL REGISTRARS OF VITAL STATISTICS IS ACCEPTABLE.
NOT ACCEPTABLE AS PROOF OF BIRTH: Baptismal Certificate, Certificate of Blessing, Certificate of Dedication, etc., Hospital Certificate, photocopied records.
